We have provided the forms necessary for scheduling an appointment under the Client Information Section. You will find Client Waiver Form; Policies & Procedures and Massage Client Intake Form. We have also provided a link below "What to Expect" for any questions you may have for before, during and after your massage.
Once you have completed the three forms, we will contact you to review your information, discuss any concerns or questions you may have and develop a protocol plan prior to your appointment. This provides you the best possible session and focus on your areas of concern as well as an understanding of your history to provide optimum results. You will receive an email confirming receipt.
