Cancellation Policy: Our time together is important. Unless there is an emergency, it is requested you cancel your appointment 24 hours in advance or pay the missed appointment fee in full. Call and leave a message on the answering machine to cancel appointments or change scheduling.
Lateness Policy: The therapist will arrive 10 minutes prior to your scheduled appointment. We request that you are prepared for your appointment upon therapist’s arrival. Our massage therapy policy states: If your appointment begins late due to the client not being ready, the time left will be used to its best advantage. If the massage therapist is late for the appointment, the remaining time will be used and the client will receive a discount or will not be charged for the massage session.
Sickness Policy: PureHealth4Life recognizes that both massage therapists and clients are vulnerable to infections and therefore, ask clients to cancel appointments when they are feeling unwell. If you have any of the following contagious illnesses, the massage session will be rescheduled: Diarrhea, Vomiting, Fever, Chicken Pox, Measles, Mumps, Meningitis, Hepatitis A, Conjunctivitis, Rubella, Head Lice, Impetigo, Influenza, Meningococcal Disease, Polio, Ringworm of the body, feet or scalp, Scabies, Thrush, Whooping Cough and the Common Cold.
Payment: Sessions will be paid by cash or check prior to the massage session. If a check bounces, the client will pay a $20 service fee, as well as any additional fines the therapist may incur as a result. If client is using insurance coverage, client will pay all session fees and bill the insurance company directly.
Sanitation and Hygiene: Our massage therapy policies on sanitation are that all equipment and linens that come in contact with the client will be cleansed in an approved method. All lubricants will be dispensed in a contamination-free manner. Before and after each massage, the massage therapist’s hands and forearms will be washed with hot soapy water and a disinfecting solution. Any breaks in the skin of the massage therapist will be covered with protective coverings. The client will also be clean for the appointment. The client will inform the massage therapist of any breaks in the skin, and these areas will be avoided. Should the massage therapist or the client contract a contagious disease that could be spread during the massage session, such as a cold, flu, or other droplet or airborne infection, each will inform the other. The decision to reschedule will be handled on a case-by-case basis.
Professional Boundaries: In order for PureHealth4Life to best serve your needs – honest disclosure of any situation that may interfere with a safe, nurturing, nonjudgmental, nonsexual touch must be considered. The massage therapist considers personal needs, biases, fears, concern, and limitations as stated herein the massage therapy policies and procedures. No food, drugs or alcohol are allowed at any time.
Respect of Client Boundaries: Swedish massage practiced on a massage table with the client undressed to comfort. The client’s body is professionally draped so that only the area being massaged is exposed. Requests for sexual activity will not be tolerated, will be viewed as solicitation, and reported to the proper authorities under the guidelines of the massage therapy policies and procedures. The client will not be rescheduled if this occurs. Room doors will never be locked. The client may stop the massage at any time and is free to leave. The client may refuse any massage methods. Children, under the age of eighteen, will not be seen for massage without a legal guardian present for the full session. Visitors may be present during the massage at your request. Please, no children in the room during massage sessions.
Confidentiality and Conversation: The discussion between the massage therapist and the client is confidential. The client may or may not choose to talk during the massage. It is inappropriate for a massage therapist to discuss the therapist’s personal life with the client. We are happy to listen to your conversation and share our professional expertise. We prefer to not discuss topics of a political or sexual nature.
Client Medical Disclosure: PureHealth4Life respects your privacy and understands that medical information about you and your health is personal and sensitive. Generally, we are required by law to ensure that medical information that identifies you is kept private. PureHealth4Life must follow both federal and state law when using and disclosing your medical information. In cases where both federal and state law gives similar protection, PureHealth4Life generally follows the law that gives greater protection of your rights, or privacy of your medical information. Some examples are laws that provide special protections for medical information about mental health, alcohol and drug abuse, HIV/AIDS, and Sexually Transmitted Disease. Your medical information may be used or disclosed for treatment (i.e., doctors, nurses, technicians, medical students, or other authorized personnel who are involved in your healthcare); payment (i.e., yourself, third party, or health insurance provider about treatment you are going to receive to obtain prior approval or to determine if your insurance plan will cover it); Appointment Reminders by contacting you in writing or by phone to remind you that you have an appointment; Individuals Involved in your Care, or Payment for your Care that we may need to release or disclose medical information about you to a friend or family member who is involved in your health care, or to someone who helps pay for your care; As Required by Law we will disclose medical information about you when required to do so by federal, state or local laws or regulations; Legal Proceedings or Law Enforcement may request medical information about you in the course of any legal proceeding in response to an order of a court or administrative agency and, in certain cases, in response to a subpoena, discovery request, or other lawful purpose.
Existing and New Medical Conditions: It is the responsibility of the client to keep the massage therapist informed of any medical treatment currently being taken, and to provide written permission from the physician, chiropractor, physical therapist, etc. that the massage may be continued. The client must provide medical information that is correct and up to date. The client must also keep the massage therapist informed of any changes in health conditions.
Informed Consent: Prior to each massage appointment, a session plan will be discussed with you. While the therapist may assist in relief of physical or emotional symptoms, the therapist does not diagnose, treat, or prescribe for any illness, ailment, or disease. This is a statement of consent that you have read the policy information, understand it, and agree to comply with the professional massage therapy policy and procedures.
Electronic Signature Agreement: This service is provided to eliminate unnecessary use of paper. By typing in your name in the signature field, you are signing this Agreement electronically. You agree your electronic signature is the legal equivalent of your manual signature on this Agreement. You consent to be legally bound by this Agreement's terms and conditions, acceptance and agreement as if actually signed by you in writing. You also agree that no certification authority or other third party verification is necessary to validate your E-Signature and that the lack of such certification or third party verification will not in any way affect the enforceability of your E-Signature or any resulting contract between you and PureHealth4Life. You further agree that each use of your E-Signature in obtaining PureHealth4Life service constitutes your agreement to be bound by the terms and conditions of PureHealth4Life Policies, Procedures and Agreements as they exist on the date of your E-Signature.
